In this month’s Case Report section of Pediatrics, Gelfand and Goadsby (doi: 10.1542/peds.2011-2624) bring to our attention a rarely noted symptom in adolescents —“primary sex headaches”. These headaches are not defined as those that interfere with sex – i.e. “not tonight I have a headache”, but a well-reported symptom in adults noted in the literature only a few times in the adolescent. A primary sex headache is either a thunderclap headache occuring simultaneously with orgasm or one with slower onset occurring in the buildup to climax. The authors note that primary sex headaches are usually benign but caution that the symptom may be secondary to a serious problem including intracranial vascular anomalies which warrant diagnosis. Once the benign nature of the headache is confirmed, treatment with indomethacin in the half- hour before sex occurs may be effective in avoiding this symptom.
